View, copy and correct the details in a youth care record

If your child receives youth care, the care worker or the institution will draw up a youth care record. The record contains information on the youth care that your child is receiving, such as treatment plans and examination reports. You can view the youth care record or request a copy of it. You can also have the youth care record updated.

Conditions for viewing a youth care record

Viewing your child’s youth care record is subject to certain conditions.   For instance, for children 16 years of age or older, you will need their consent.

Correcting the details in a youth care record

If the youth care record contains incorrect information, such as the wrong date of birth, please request a correction. Or request destruction of the information in the youth care record. The youth care worker or youth care institution will determine whether this is possible.

Submit a request to view, copy or correct the record

If you want to view a youth care file, request of copy of it or have certain details corrected, you must request this in writing. Please submit your request to the youth care worker or youth care institution employee.
