COVID-19 and flying to the Netherlands: health declaration form (English and Dutch)
All travellers aged 12 or older who are flying to or from the Netherlands must carry a health declaration. The declaration states whether you have any symptoms of coronavirus infection (COVID-19). You must complete the declaration before your flight. If you have no symptoms you may board the aircraft.
You can print the health declaration and fill it in on paper, or you can complete it digitally on your mobile phone or tablet.

Printed health declaration
If you want to fill in the health declaration on paper, print 2 copies. You will need one for your outbound flight and one for your return flight.
Health declaration on your phone or tablet
Check that the digital health declaration works
You can download the health declaration as a PDF that you can fill in digitally and save on your mobile phone. Not all mobile phones and tablets support fillable PDFs, however. Before you go to the airport, check that you can fill in and save the PDF on your phone or tablet.
Print the declaration if you cannot fill it in digitally
If you downloaded the PDF on your phone or tablet but you can’t fill in the declaration digitally, print 2 copies for yourself and for anyone else you are travelling with. You will need one for your outbound flight and one for your return flight.
Printing or downloading declaration not always necessary
If you’re flying with KLM, Corendon, TUI, Transavia or easyJet, you will be asked to fill in the health declaration as part of online check-in procedures. So you don’t have to print or download it.
Show the health declaration when boarding
You need to show the completed health declaration when you board the aircraft. You can also be asked to show your health declaration in other parts of the airport. So make sure you have the declaration with you at all times.
