Annex 5 - Track Record

Please note:

• Annex 5 will be used to verify that the applicant meets the threshold criteria.
• If you are applying to two regions (for the instruments CVW-WHRD and WPS), the information provided in Annex 5 must cover both regions and D3b should relate to all countries in the region(s). Annex 5 needs to be submitted only once.
• You may copy and paste the table if additional project references are needed.

The information in the table should be presented concisely. A qualitative assessment of the applicant’s track record will be carried out through the project proposal format (Annex 3.1, 3.2, and 3.3).
